Eczema or ringworm? How to tell the two rashes apart

You have a round-ish, itchy patch on your skin, and every picture you search seems to show both a ring and a rash. Eczema or ringworm?

Here is what each one tends to look like up close, six checks you can do with a mirror and a fingertip, and the one cream mistake that can make ringworm worse.

Quick answer: Ringworm tends to have a sharp, raised, scaly edge, often with a smoother middle, and it creeps outward. Eczema tends to have blurry edges and stays scaly all the way across, often on both sides of the body and in skin creases. Ringworm is a contagious fungus. Eczema is not. One look-alike, round "coin" eczema, fools a lot of people, so a patch that does not fit cleanly is worth getting looked at.

What eczema looks like

An eczema patch is dry, scaly and itchy, and its edge fades into the surrounding skin instead of stopping at a line. On light skin it looks pink or red. On brown and Black skin it more often looks darker brown, purple or gray, so go by the rough texture more than the color.

It likes creases: the inner elbows, the backs of the knees, the neck, wrists, hands and eyelids. It also tends to show up on both sides, so a patch on one inner elbow often has a twin on the other.

The itch is the main event, and it is often worse at night. Scratch long enough and the skin can ooze and crust, or thicken into darker, leathery patches.

Eczema usually starts in childhood and travels with asthma, hay fever or other allergies, in you or your family. It flares with dry air, harsh soap and detergents, scented products, wool, sweat and stress, then settles. It is not contagious.

The look-alike to know: nummular eczema forms round, coin-shaped patches, and it is the one that gets mistaken for ringworm. The tell is that it is usually scaly or crusted evenly across the whole coin, with no smoother middle and no raised ring for an edge.

What ringworm looks like

There are no worms. Ringworm is a fungal infection of the skin.

The classic patch is round or oval with a raised, scaly edge you can feel with a fingertip. As it grows outward, the middle often calms and looks smoother, and that is what makes the ring. Not every patch does this: some stay solid and scaly all the way across.

On light skin the edge looks pink or red. On brown and Black skin it can look darker brown or gray, and the color may be hard to see at all.

It usually starts as a small spot and creeps outward. You often have one patch or a few, scattered unevenly rather than mirrored. It may itch, or hardly at all.

You catch it from skin contact with someone who has it, from shared towels, clothes, gym mats and sports gear, and from pets. Cats, dogs and farm animals can carry it, kittens especially.

It is the same family of fungus that causes jock itch and athlete's foot. On the scalp, especially in children, it needs a doctor and usually prescription tablets. A doctor confirms ringworm by scraping a little skin and looking at it under a microscope or sending it for culture.

How to tell them apart at home

Trace the edge with a fingertip. A raised, defined border you can feel points toward ringworm. An edge that fades away gently points toward eczema.

Look at the middle. A smoother, calmer center inside a scaly ring leans ringworm. The same scaly texture all the way across leans nummular eczema, though some ringworm patches look like that too.

Check the other side. A matching patch on the other elbow or knee leans eczema. A lone patch leans ringworm.

Think about your life, not just your skin. A new kitten, a gym mat, a wrestling team or a shared towel lean ringworm. Hay fever, asthma and a lifetime of dry, itchy skin lean eczema.

Watch it for a few days. Ringworm creeps outward at the border. Eczema tends to flare as a wider, vaguer area.

Notice what plain moisturizer does. Eczema often feels calmer and less tight after a few days of thick moisturizer. A patch that keeps widening anyway is acting more like a fungus. Treat this as a clue, not a test.

Some round patches simply cannot be told apart by eye. If yours is one of them, a skin scraping is how a doctor settles it.

What helps each, and when it backfires

Talk to a doctor or pharmacist before you start anything, especially in pregnancy, while breastfeeding, if you take other medicines, for babies and young children, or with kidney or liver disease.

For eczema:

  • A thick, unscented cream or ointment every day, applied soon after a short lukewarm bath.
  • Cutting back on triggers: harsh soap, scented products, wool, long hot showers.
  • Over-the-counter hydrocortisone 1 percent for short courses on body flares. Not for long stretches, and not on the face or groin without a doctor's advice.
  • Stubborn eczema has prescription options, so you do not have to live with it.

For ringworm:

  • An over-the-counter antifungal cream (clotrimazole, miconazole or terbinafine are the usual active ingredients), applied to the patch and a little past its edge.
  • Use it for the full time on the label, which is often some weeks, and keep going after it looks better, or it tends to return.
  • Wash towels and clothes hot, do not share them, and have a vet look at a pet that might be the source.

Where it backfires:

  • Steroid cream on ringworm. It can calm the itch and redness while the fungus keeps spreading, and it can blur the ring so the patch is harder to recognize later. Doctors call this tinea incognito. It is the main reason not to throw hydrocortisone at a round patch you cannot name.
  • Antifungal cream on eczema. It does nothing for eczema, and creams can sting dry, cracked skin.
  • Scratching either one. Broken skin invites a bacterial infection.

When to see a doctor

  • The patch has not improved after the full label course of an antifungal cream.
  • It is spreading fast, or you have several new patches.
  • A scaly patch is on the scalp or beard area, or you notice nail changes.
  • You have diabetes or a weak immune system, or you take steroid or immune-suppressing medicine.
  • A baby or child has a scaly patch on the scalp.
  • Eczema turns cracked, weepy, crusted with yellow scabs or very painful, which can mean infection.
  • You used steroid cream and the patch got bigger, blurrier or changed. Tell the doctor exactly what you put on it.
  • The itch is wrecking your sleep, or moisturizer and trigger changes are not enough.

Yes, it spreads by skin contact and shared towels, clothes and gym gear. Many schools and workplaces allow attendance once treatment has started and the patch is covered, but rules differ, so ask a doctor or pharmacist and check the local policy.

Why did my rash get worse after steroid cream?+

If the rash was ringworm, steroid cream can quiet the inflammation without killing the fungus, which keeps growing and can look different. Tell a doctor which cream you used and for how long.

How long does ringworm take to go away with cream?+

Many people see it ease within the first couple of weeks, but the full label course is usually longer. Stopping early is a common reason it comes back.
